HOW TO APPLY FOR UTAH DISABILITY BENEFITS

Our law office address is 633 East South Temple, Salt Lake City. It is easy to find. From either the north or south entrance on I-80, take the 6th South Exit. Then, drive east seven blocks until your reach 7th East. Turn left and drive 7 blocks to South Temple. Turn left once you reach South Temple and our office is on the right. Our office building is directly across the street from the Masonic Temple.
ACCESSIBILITY TO OUR LAW OFFICE
The entrance door to our building is not on South Temple. It is on H Street. Please pull onto H street and park on H street, as you see in the above photo. There is ample free parking on the street on H street, which is to the west of our building. Or, you can park in our parking lot behind the building.
You will know which door is our front door because there is a water feature in front of it. You can use a ramp or the stairs to get to the front door. Don’t walk by the water feature and try to enter the building on South Temple. We have blocked that entrance to the building. It is only used as an emergency exit door.
If you have a wheelchair, you can use our parking stall in our free parking lot. We also have a ramp to our front door, which is helpful if you use a walker or cane. Inside we have a comfortable waiting room. Additionally, we have bathrooms with wide doors and hand bars, so they are easy to use for those with a walker, cane, or other helpful device.
APPOINTMENTS
We love it when our clients visit our office, but we do ask that you make an appointment. Also, it is a good idea, when dropping off medical records or paperwork for an appeal or application, to call first. That way, our staff can make time to copy your paperwork and return the original to you. It is our policy to never keep your original paperwork in our office. We prefer to scan it directly into our case management system and return your original copy. Therefore, if you bring in your medical records, then plan to wait in our office for at least 15 minutes while we scan your paperwork.
Additionally, please note that we usually close early on Friday afternoons for staff training. Also, sometimes we close because we are having a full day of training. Full day training usually happens once a year. Even so, it is best to call before you come to the office. That way we know you are on your way.
Additionally, we are closed on all federal holidays. So, before you come to the office, make sure one of our staff members know you are coming. Also, if you let us know you are coming in, then we can make sure there is a conference room available for you to meet with us.

However, if you have a medical condition that automatically wins SSD benefits, you should file your application as soon as possible. Once you submit your application online, the SSA sends you an application summary in the mail. You must sign the summary and mail it back. Send it in quickly.
